The following are policies of Forever Dance, LLC and are intended to clearly communicate expectations for both you and your child in order to ensure a safe, positive learning experience:
Code of Ethics: Each Forever Dance student is committing themselves to improve and perfect their dance technique and style, as well as serve as a role model to all other dancers here and in the community.
Payment: Payment is due in advance on the 15th of each month. No exceptions will be made to this policy. For example, May payment is due on April 15th. There will be a $25 late fee added to the account for each payment received after the 15th. For accounts more than 30 days past due, the student will be suspended until the account is current. A written 30 day notice must be given to the owner if the student is not going to complete the full term. If the student begins classes in the middle of a month, that month will be pro-rated and the next month’s tuition will be due along with the pro-rated month’s tuition at the time of Registration.
All costume fees must be paid in advance. All costume fees and deposits that are paid after the due date will be subject to a $10.00 late fee. Any costumes not paid in full will result in that child not performing in the recital. Costume fees are non-refundable.
All Recital Fees must be paid in advance at time of Registration and are non-refundable.
Dress Code for 2008-2009 Dance Year:
All Forever Dance Students are required to purchase Leotards, tights, and shoes from us so that everyone will match accordingly.
3 & 4 year olds are required to have pink leotards and pink tights with pink canvas ballet slippers for ballet and Tan Tap Shoes for tap.
5 & 6 Yr. olds are required to wear lavender Leotard and Pink Tights with pink canvas ballet shoes for ballet and tan tap shoes for tap.
7-10 yr. olds are required to wear light blue Leotard and Pink Tights with pink canvas ballet shoes for ballet, tan tights and tan tap shoes for tap, and tan tights and tan jazz shoes for jazz, tan footless tights and dance paws for lyrical, Light Blue Leotard and tan tights and light blue soffee shorts and black hip hop shoes for hip hop.
11-14 yr. olds are required to wear forest green Leotards and pink tights with pink canvas ballet shoes for ballet, tan tights and tan tap shoes for tap, tan tights and tan jazz shoes for jazz, tan footless tights and dance paws for lyrical, forest green leotard and tan tights with black soffee shorts and black hip hop shoes for hip hop.
Dance Company Members are to wear black leotard w/FD Logo and pink tights with pink canvas ballet slippers for ballet, tan tights and tan tap shoes for tap, tan tights and tan jazz shoes for jazz, tan footless tights and dance paws for lyrical, and tan tights and black soffee shorts with black hip hop shoes for hip hop.
No Skirts, Sweaters, Sweatshirts, pants, shorts, etc.. may be worn in class. There will be tests in Ballet for students to get the privilege of putting ribbons on their shoes or wearing a skirt to class.
Makeup classes: Makeup classes will not be available unless the class is cancelled by the teacher. If your child misses a class, payment is still due and the child must make up any missed material on their own. If an injury does not allow participation, please come and watch if possible. We break for Christmas, Spring Break, Thanksgiving, and July 4th. Those are considered “holidays” and are not made up as missed classes.
Class times: Your child may not be allowed to attend class if they are more than 10 minutes late. The owner will use discretion in making exceptions to this policy depending on the circumstances in each case. If your child is consistently late they will be suspended for one week. It is extremely important to make every effort to get your child to class on time.
Because the Dance Studio set up is not ideal to provide waiting space, parents are discouraged from staying during class. It is distracting to the children and makes teaching more difficult. Please plan to drop your child off and return when the class is over.
You are responsible for picking up your child at the conclusion of each class. Forever Dance cannot be responsible for children left on the premises for more than 15 minutes after their class ends.
Attendance: Because your child will be learning with a group of other students, all students are expected to attend every class. No more than 3 absences will be allowed during each term. This should accommodate family vacations, illnesses, or other circumstances. In fairness to the group, if your child misses more than 3 classes during a term, they will not be allowed to perform in the recital or in any other public performances given by Forever Dance. The owner reserves the right to use discretion in the enforcement of this policy due to individual circumstances. If you are having difficulty with lateness or attendance, it is important that you keep the owner informed as to any extenuating circumstances.
Behavior: In order to provide a positive atmosphere for all students, any behavior that is deemed to be inappropriate will be brought to the attention of the offending student. Any student who shows disrespect toward the teacher, has excessive absenteeism, uses profanity, or fights and/or argues with the teacher or other students will be removed from class, up to and including permanent dismissal from the studio, based on the severity of the behavior problem. No refunds of any form are given to students that were permanently removed from the studio.
Behaviors that are considered inappropriate include but are not limited to: disrespectful attitude towards the teacher or other students, gossiping, talking back, chewing gum, or using cell phones. Please take steps to ensure that your child’s cell phone is turned off while they are at the studio. Cell phones will be confiscated from students that violate this rule and will be returned only to the parent.
Choreography is at the sole discretion of the owner and/or class teacher. Neither you nor your child should attempt to advise the teacher how to choreograph any number.
Health concerns: The owner must be kept informed of any health issues your child is experiencing. For chronic disorders such as asthma, diabetes, allergies, hypoglycemia, etc. medication labeled with your child’s name and instructions for treatment should be left with the teacher so that issues experienced during a parent’s absence can be addressed immediately for the safety of the child. For physical disorders such as pulled muscles, sprained joints, broken bones, etc. a Doctors note certifying that it is OK for your child to dance is required.
Insurance: Forever Dance is not liable for any injuries or other damage experienced at the studio or in any other location where performances or appearances may be held. Students must have their own health insurance in the event that treatment becomes necessary for any reason.
Hours: Business hours are Monday through Friday 10:00AM until 2PM. If it is necessary for you to call outside of these hours, you will reach voicemail and your call will be returned during business hours as the class schedule allows. In general, phone calls are not answered while class is in progress. Please be considerate of the teachers and students and only contact the studio when necessary.
